News From the Nurse
As the school year proceeds, I’m going through files to be sure all immunizations are up to date. I’ll be notifying you by phone or mail if I see something that needs attention. If you choose to not vaccinate your student for any or all diseases, please be sure we have a Refusal of Immunizations form, signed by you and notarized in the file. This is a state requirement. If you’ll notify the school, I’ll send you one.
We’re seeing an increase in allergies and asthma and if your student has an allergic reaction to something that could trigger a reaction, or if they have asthma, we’d like to be aware so we can respond appropriately. I have a helpful form you can easily fill out identifying the allergen and reaction. Also, if your student has an epi-pen we need a medication form filled out. Please call or stop by the school and pick up any of the forms you may need.
If any medications are to be given at school, we need a signed release submitted to the nurse. (The office will put it in my mailbox.)
I’ll be notifying you when I’ll be doing height and weights, Lyons Club visit for vision and hearing exams and the dental hygienist visit. Please be sure and notify the school if you’ve moved and have a new address or if your phone number has changed so we can get hold of you regarding your student if needed.
